KINDERGARTEN - 8TH GRADE ENROLLMENT

PLEASE CALL THE SCHOOL OFFICE BEFORE COMPLETING THE ENROLLMENT FORMS.  SEATS ARE LIMITED IN EVERY GRADE AND ARE NOT GUARANTEED.  ENROLLMENT MAY NOT BE ACCEPTED UNTIL YOU HAVE CONTACTED THE OFFICE TO DETERMINE ELIGIBILITY AND IF A SEAT IS AVAILABLE IN YOUR CHILD'S GRADE.

New family registration for the 2025-2026 school year will open on February 1, 2025.  You may complete and submit the forms below.  Once your forms are received and reviewed, we will notify you of your enrollment status.  Registrations are not complete until you have established your FACTS Management Tuition Agreement, and your registration fee has been processed.  FACTS Management application for the 2025-2026 school year is now open.

If your child is entering grades 1 through 8, a copy of their most recent report card, attendance record, and IEP/504 plan (if applicable) will be required for admission. Saint Ambrose School reserves the right contact prior school of enrollment and to administer a benchmark assessment to evaluate grade level readiness.

You must DOWNLOAD the form before filling in the fields.  Once you have completed filling out the forms be sure to save each form with your family last name at the end of the document name.  Once all forms are complete, email them to [email protected].  Please send PDF documents (not JPEG's).  

PLEASE BE AWARE THAT REGISTRATIONS WILL NOT BE ACCEPTED WITHOT THE FOLLOWING REQUIRED DOCUMENTS:

1. Birth Certificate

2. Sacramental Certificates (if applicable)

3. Most recent immunization record.

4. Universal Health Recorded completed and stamped by the doctor.

Please be aware that each family is required to maintain a FACTS Management tuition payment account.  Once you have emailed all of the documents below be sure to establish your FACTS Management account (link below).  Once you establish your account, your registration fee will be processed and you will receive an email indicating that the registration is complete.
