Enrollment Forms

New family registration for the 2026-2027 school year is now open.  Please call the office first to determine if there are seats available in your grade level. You may complete and submit the forms below.  Once your forms are received and reviewed, we will notify you of your enrollment status.  Registrations are not complete until you have established your FACTS Management Tuition Agreement, and your registration fee has been processed. Please be aware that registration fees and school fees are non-refundable.  FACTS Management application for the 2026-2027 school year is now open.
